To get started, you must have the following information handy: |

Membership Number - This is located on your membership card. Please
disregard the last number on your card. For example, if your card has 1234560, the
membership number you need to enter is 123456. |

|
|
|
Home Phone Number - This is the most current phone number you have
in our database. If you have recently changed phone numbers, please try your previously
used number. If you do not remember the phone number that is in our database, please
contact us at 1.800.313.7208. |

|
|
Email Address - This must be a valid email address as you will
receive a pin number upon pressing the "submit" button below.* If you do not receive
an email within 1 hour, please contact us at 1.800.313.7208 as your email address
may reject unknown senders or there may be a spelling error. |

After gathering this information, please enter it below and press the "Submit" button.
Upon successful submission, you will receive an email to the address provided, giving
you a "secret pin" to input with your membership number to view your online statement
and account information. |
